The haemodynamics of mitral stenosis before and after commissurotomy.

چکیده
Clinical changes following mitral valvotomy are sometimes difficult to evaluate. The need for an objective method in assessing the effectiveness of surgery is apparent, and the value of combined heart catheterization in appraising the hemodynamics of mitral stenosis has been reported (Goldberg et al., 1955, 1956). A total of 58 patients have been studied by this procedure, 16 of them after as well as before mitral valvotomy. The importance of the combined technique can be readily appreciated, particularly in assessing the changes of pressure gradient and flow across the involved valve. Comparison of the data with those previously obtained by right heart catheterization alone has yielded significant differences and provides additional support for the continued use of this new technique.

SHORT TERM CLINICAL OUTCOME OF PERCUTANEOUS BALLOON MITRAL VALVULOPLASTY VERSUS SURGICAL CLOSED MITRAL COMMISSUROTOMY
BaIlon mitral valvuloplasty and closed mitral valve conunissurotomy were performed in 450 and 127 patients, respectively. Also, 254 patients with severe mitral stenosis were enrolled in a prospective randomized trial comparing the two procedures. Immediately after balloon mitral valvuloplasty, mean left atrial pressure was 11.38±3.54 mmHg, transmitral valve gradient was 1.8±2 mmHg, and mit...
متن کاملThe Association between B-type Natriuretic Peptide Family and Successful Percutaneous Transvenous Mitral Commissurotomy: A Systematic Review
B-type natriuretic peptide (BNP) level is known to increase in patients with rheumatic mitral stenosis. In this systematic review, we aimed to discuss the possible association between plasma BNP level and the success rate of percutaneous transvenous mitral commissurotomy. PubMed and Scopus databases were searched systematically, using the following key terms: “B-type natriuretic peptide” OR “BN...
متن کاملEffect of uterine contractions on left atrial pressure in a pregnant woman with mitral stenosis.
Several studies of peripartum haemodynamics in normal pregnancies have been performed.1-3 The results, however, are somewhat confusing, probably because of methodological factors and the use of indirect methods to assess haemodynamic variables.3 We are aware of only one study on peripartum haemodynamics in parturients with mitral stenosis in which a pulmonary artery catheter was used, but measu...
متن کاملPlasma concentrations of adrenomedullin correlate with the extent of pulmonary hypertension in patients with mitral stenosis.
OBJECTIVE To examine the pathophysiological significance of adrenomedullin in the pulmonary circulation by investigating the relation between plasma concentrations of adrenomedullin and central haemodynamics in patients with mitral stenosis. METHODS Plasma concentrations of adrenomedullin in blood samples obtained from the femoral vein, pulmonary artery, left atrium, and aorta were measured b...
متن کاملResolution of massive left atrial appendage thrombi with rivaroxaban before balloon mitral commissurotomy in severe mitral stenosis
RATIONALE Data on nonvitamin K antagonist oral anticoagulant being used for the treatment of LAA thrombi are limited only in nonvalvular atrial fibrillation. There are no data on the antithrombotic efficacy and safety of nonvitamin K antagonist oral anticoagulant in the resolution of left atrial appendage (LAA) thrombi in patients with rheumatic mitral stenosis. PATIENT CONCERNS A 49-year-old...
